Questions in an NCLEX-RN exam basically comes from four sections i.e. Physiological Integrity Safe, Psychological Integrity, Effective Care Environment, and Health Promotion and Maintenance. Questions in this section deals with psychiatric problems, organic mental disorders, crisis intervention, substance abuse, anxiety disorders, schizophrenia. Approximately 12 percent of questions come from the Psychological Integrity section in the NCLEX-RN test.
Questions related to gerontology, pediatrics, adult medical care, surgical care, diabetes, respiratory diseases, neurological disorders, renal diseases, skin disorders, and infectious diseases are asked in this category. The majority of questions i.e. 43% to 67% questions fall under this category.
Questions in the Safe and Effective Care Environment deals with patient care, prevention of injuries and infections, pediatric patients’ safety, and laboratory test results. This section contains from 21% to 33% of NCLEX-RN questions.
Questions in the Health Promotion and Maintenance deals with pregnancy, labor, delivery, infant care, pregnancy symptoms, pregnancy complications, pregnancy prevention, birth control measures and sexually transmitted diseases. This section contains approximately 12% of the NCLEX-RN questions.

The computer will generate the questions based on how you answered the previous question. The time duration of the NCLEX-RN exam is five hours and after 2 and a half hours, participants will be given a mandatory 10-minute break. NCLEX-RN exam scores are determined by comparing how you responded to the questions.
